Isshiki's history on Earth began when he and Kaguya arrived millennia ago, only for her to betray him. Left behind in a dying state within the dimension she created, Isshiki survived by shrinking his body to minimize energy consumption. He remained trapped there until the modern era, where he found Jigen, a human who became his vessel. Through Jigen, Isshiki operated as the hidden leader of Kara, an organization searching for people with Karma marks—seals that allow Otsutsuki to resurrect via a host's body. Amado revealed that all Karma users, including Boruto and Jigen himself, would eventually be taken over by the Ōtsutsuki clan. Isshiki's ultimate plan involved finding Kawaki, another Karma host, to fully restore his physical form and sacrifice humanity as fertilizer for a new God Tree.

In Their Own Story

Isshiki's return to the modern world began when he broke free from Kaguya's dimension and possessed Jigen. Under this disguise, he manipulated Kara to locate suitable hosts with Karma marks, specifically targeting Kawaki. When Naruto confronted him directly, Isshiki attempted to reclaim his full power but was hindered by his limited lifespan as a result of the long years spent in a shrunken state. Despite his efforts, Isshiki could not fully sustain himself and eventually died after failing to secure Kawaki as a permanent vessel. Before his death, he instructed Code, the guardian of the Ten-Tails, to carry on the Ōtsutsuki will. This directive set in motion the release of Eida and Daemon, two powerful cyborgs created by Amado from the remains of Shibai Ōtsutsuki, ensuring that even after Isshiki's demise, the Otsutsuki agenda would continue through new means.
