The origin of the Otsutsuki on Earth traces back to Kaguya Ōtsutsuki, who arrived millennia ago. She was eventually betrayed by her successors, leading to her being sealed away. Centuries later, Momoshiki and Kinshiki Ōtsutsuki returned, abducting Naruto Uzumaki to use Kurama as fuel for their dying Divine Tree. Before his death, Momoshiki placed a mark called "Karma" on Boruto Uzumaki. Amado revealed that Karma allows the clan to resurrect via a host's body. This mechanic was central when Isshiki Ōtsutsuki possessed Jigen, who had been under his control since Kaguya's betrayal. The cycle of resurrection became critical when Momoshiki used it to possess Boruto after Kawaki killed him. Additionally, Amado explained that Shinjutsu are divine abilities more powerful than ninjutsu, transplanted from the remains of Shibai Ōtsutsuki, an Otsutsuki who achieved godhood and transcended to another plane. The clan's hierarchy includes guardians like Code, who was tasked by Isshiki to carry on their will after his death. Code later released powerful cyborgs Eida and Daemon, created from Shibai's remains, to further the clan's objectives against the ninja world.

In Their Own Story

Momoshiki and Kinshiki abduct Naruto Uzumaki to revitalize the dying Divine Tree in their dimension. Boruto, Sasuke Uchiha, and four Kages rescue him, defeating the Otsutsuki brothers. However, before dying, Momoshiki places a Karma mark on Boruto. Later, Jigen, revealed to be possessed by Isshiki Ōtsutsuki, hunts for Kawaki, another vessel. Isshiki confronts Naruto head-on but dies due to his limited lifespan. Seizing the moment, Momoshiki attempts to possess Boruto and kill everyone else until Boruto regains control. Isshiki then orders Code to continue their mission. Code releases Eida and Daemon, cyborgs with Shinjutsu powers. Eida agrees to help Code kill Naruto if he spares Kawaki, whom she wishes to romance due to her seduction powers being hindered except by Otsutsuki. In the climax of Part I, Momoshiki takes full control of Boruto's body, forcing Kawaki to kill him. Momoshiki revives Boruto, turning him into a full Otsutsuki. Kawaki then sends Naruto and Hinata to another dimension, promising to free them only after killing Boruto. Using Eida's Omnipotence, Kawaki rewrites memories so the world believes Boruto killed Naruto and Hinata, framing him as a criminal while he vows to regain everything taken from him.
